International Students use this form to request an I-20 to use in applying for Optional Practical Training

Allow a minimum of 7 working days for an OPT I-20 to be prepared. You will receive an e-mail once it is available for you to pick-up. Your OPT application must be received by USCIS within 30 days of the date on your OPT I-20.

Please list your personal phone number where you can be reached throughout your OPT. This is how I will contact you once your OPT period begins. It is very important that I be able to contact you while you are OPT as I will still be responsible for maintaining your SEVIS record.
